Essential Lifestyle Planning

Essential Lifestyle Planning: Process Facilitation and Plan Writing

Essential Lifestyle Planning is a person-centered process intended to change the way we plan, design, and deliver supports. This workshop is intended for those interested in understanding all elements of the Essential Lifestyle Planning process. Participants will learn the process of gathering information to produce powerful and effective support plans.

Prerequisite: Completion of Person Centered Thinking

Essential Lifestyle Planning: Facilitator Certification

Essential Lifestyle Planning is an approach to person-centered planning intended to change the way we plan, design, and deliver supports for people with intellectual and developmental disabilities who are being supported in their communities. This workshop is required for final certification for social workers, case managers, supports coordinators, provider agency staff, families, and others in order for them to facilitate and write Essential Lifestyle Plans. Participants will learn how to effectively self-assess plans, develop strategies for implementation and on-going review, and learn to utilize specific tools for focusing on planning to generate outcomes for community connection and participation.

Prerequisite: Completion of Essential Lifestyle Planning: Process Facilitation and Plan Writing. Registrant must facilitate and submit a completed Essential Lifestyle Plan for review.

Registration Details

NOTE: Those interested in certification as ELP Facilitators must complete Person Centered Thinking, ELP Process Facilitation and Plan Writing, and ELP Facilitator Certification training, a total of five training days.  If you are pursuing ELP Facilitator Certification, you should also register for the ELP Facilitator Certification workshop.

Fee information: Employees of DDD, staff of agencies funded by DDD, as well as individuals with disabilities and family members - no charge. All other registrants - $150 for ELP Process Facilitation and $75 for ELP: Facilitator Certification.

Please note: Submission of the online registration form sends an email request to The Boggs Center. Submission of the online form is not a confirmation of registration. When your form is received, Boggs Center staff will register you for the course requested, based on space availability. If the training is full, you will be notified. Written confirmation of your registration and materials will be mailed two weeks prior to the training course. If you do not receive a confirmation, please call us to verify registration and to receive directions and materials.
